Student who repeat a course where the original grade was an "F" according to the repeated course policy should contact the Office of Academic Services if:

• The course was taken with a different course number (for example: Calculus I 01:640:151 repeated with 01:640:135)

Please download the Application for Repeated Course with Original Grade of "F" form and return it to any Academic Advising Center.

If the course was repeated with the same course number, there is no need to fill out this form.
